It sounds like you've got the bases covered; usually when this problem
comes up, it is a problem with one of the files you mentioned.  So
nothing really comes to mind.  Keep in mind that name resolution is
generally done by a library (libc or whatnot), so you probably need to
look there.  To help with your quest, consider doing an strace or
similar trace to see what files are opened and functions called during
the process.  Maybe something will become obvious.
